Tarsal tunnel syndrome is essentially carpal tunnel for the foot, compression of the tibial nerve as it passes through a narrow tunnel on the inside of the ankle. Symptoms include burning, tingling, or shooting pain from the inner ankle into the foot. It’s often misdiagnosed as plantar fasciitis or neuropathy.

A tarsal tunnel visit is primarily about getting the diagnosis right, this condition is often confused with plantar fasciitis or general neuropathy.

Tarsal tunnel causes burning, tingling, or electrical sensations, signs of nerve involvement. Plantar fasciitis causes deep aching pain in the heel that’s worst with the first steps of the day.

Sometimes, if the cause is temporary (swelling from an injury or pregnancy-related fluid retention). Chronic tarsal tunnel rarely resolves without treatment, especially if there’s an anatomical cause like a cyst or bone spur compressing the nerve.

A careful physical exam with the Tinel’s test (tapping over the nerve to reproduce symptoms) is often diagnostic. Nerve conduction studies and ultrasound can confirm the diagnosis and identify compressive causes that might need surgery.

For well-selected patients with a clear compressive cause, surgical success rates are 75–90% for significant symptom relief. Success depends heavily on proper patient selection and addressing any anatomical factors during surgery.

They’re similar syndromes, both involve nerve compression in narrow anatomical tunnels, but usually have different causes. Having carpal tunnel doesn’t necessarily mean you’ll get tarsal tunnel, though certain conditions like diabetes or arthritis can predispose to nerve compression at multiple sites.
