Shin splints cause diffuse pain along the inner shin; stress fractures cause pinpoint pain at a specific spot. Stress fracture pain is worse with hopping on one foot and often persists at rest.

Ice for acute flares, 15–20 minutes after activity reduces inflammation. Heat can help before activity to warm up tissue.

Mild shin splints can sometimes be managed with reduced mileage and softer surfaces. Moderate or severe pain requires time off from running to allow healing.

Some patients find compression sleeves reduce symptoms during activity, though evidence is mixed. They don’t treat the underlying cause but can provide comfort as part of a broader treatment approach.

Gradual mileage increases (no more than 10% per week), replacing running shoes every 200–300 miles, calf and hip strengthening, and running on varied surfaces. Addressing any underlying biomechanical issues with orthotics prevents the majority of recurrences.
