What to Expect at Your First Visit

Your first heel pain visit identifies the exact cause (it’s not always plantar fasciitis) and starts treatment immediately.

Plan on 45 to 60 minutes. Wear athletic clothing and bring the shoes you wear most.

Focused history of when the pain started and what makes it better or worse

Physical exam of the foot, arch, and Achilles tendon

Digital X-ray in-office to rule out stress fracture or heel spur

Diagnostic ultrasound to assess the plantar fascia or other soft tissue

Almost never. Heel spurs on X-rays look dramatic but rarely cause the pain themselves, it’s the inflamed tissue around the spur that hurts.

Overnight, the plantar fascia tightens while you’re resting. The first steps in the morning forcefully stretch that tight tissue, which is painful and can cause small re-tears.

Sometimes, but often it’s more complex. Worn shoes or wrong shoes contribute to heel pain, and switching shoes can help mild cases.

Children with heel pain often have Sever’s disease, inflammation of the growth plate in the heel. It’s completely different from adult plantar fasciitis and requires different treatment (rest, heel pads, calf stretching, activity modification).

A single cortisone injection for plantar fasciitis is generally safe and effective. Repeated injections in the same spot (more than 2–3 over time) can weaken the plantar fascia and increase rupture risk, so we use injections judiciously and explore other options when available.
