What to Expect at Your First Visit

A flat foot visit assesses whether your arch flattening is causing your symptoms and what, if anything, should be done about it.

Plan on 45 to 60 minutes. Bring the shoes you wear most often and any existing orthotics.

History of when the flat feet started and what symptoms they cause

Physical exam, including standing and walking assessment

Tests to assess the posterior tibial tendon (common source of adult flat foot)

Digital X-ray or ultrasound in-office if needed

Custom orthotic prescription if indicated

Physical therapy referral for tendon and arch strengthening

Discussion of bracing or surgery only if significantly needed

Usually no. Painless flat feet in adults generally don’t require treatment.

Yes. Adult-acquired flat foot deformity (AAFD) is most commonly caused by progressive weakening of the posterior tibial tendon.

Most children do, a true adult arch develops by age 6–7 in most kids. If a child’s flat feet are painful, limit activity, or are associated with a tight Achilles tendon, evaluation is warranted.

No. Plenty of elite runners have flat feet.

Reconstructive flat-foot surgery can produce excellent results in appropriate candidates. It’s major surgery with a long recovery (3–6 months minimum), so it’s reserved for patients with significant pain and dysfunction who haven’t responded to conservative treatment.
