Charcot foot is a serious complication of diabetes-related nerve damage, in which the bones of the foot weaken and collapse, often painlessly. Because there’s little or no pain, patients keep walking on the foot, making the damage worse. What to Expect at Your First Visit

A suspected Charcot foot visit is urgent. We aim to see these patients the same or next day because every day of walking on an active Charcot foot causes more damage.

Plan on 60 to 90 minutes. Please do not walk into the visit if you can avoid it, a wheelchair or crutches significantly protect the foot during evaluation.

Focused history of diabetes control, neuropathy, and when symptoms started

Careful exam comparing both feet (temperature, swelling, shape)

Digital X-ray to assess bone and joint changes

Advanced imaging if needed to distinguish Charcot from infection

Immediate offloading: total contact cast or specialized boot

Strict instructions on weight-bearing (usually none, initially)

Weekly follow-up plan until the foot is stable

Yes. While diabetes is the most common cause in the US, Charcot foot can occur with any condition that causes severe peripheral nerve damage, alcoholism, spinal cord injury, leprosy, or certain neurological diseases.

The acute inflammatory phase typically lasts 3–6 months with proper treatment. Total contact casting or boot wear may continue for 6–12 months before the foot is stable.

Most patients don’t, especially when caught early. Surgery is considered when the foot is significantly deformed, unstable, or developing ulcers.

Not without appropriate offloading. Walking on an acute Charcot foot can worsen the damage dramatically and quickly.

Recurrence is possible, especially if blood sugar control is poor or if you return to regular shoes too quickly. Lifelong custom footwear and close monitoring significantly reduce recurrence risk.
